The To Heart game for the PC was released in 1997 by Aquaplus, a spinoff of Leaf game company. (There are also Playstation and Sega versions.) It is a role-playing-type game, where you (as Fujita Hiroyuki) can walk around the high school and interact with the other characters. Each one has a storyline which progresses differently depending on choices you make. From what I understand, the object is to figure out how to get to the best ending with each girl. These all involve H (hentai - adult) scenes. Which is why I won't be playing the game. :P The Playstation version (released in 1999) apparently takes out the hentai scenes. In addition, the girls from To Heart are included in "Queen of Hearts," a parody game which allows all the characters from various Leaf games to fight each other Street Fighter-style (similar to Super Smash Bros. on N64, where Mario, Donkey Kong, Pikachu, etc. can fight each other).

There are slight variations in the characters from the game to the TV series. First of all, the character design is more angular. There are eight girls in the game: Akari, Shiho, Hoshina-san, Aoi, Lemmy, Serika, Kotone and Multi. Rio is a "secret" character. Masashi, Serio and Ayaka make appearances but I'm not sure about Yoshie and Okada.

A few of the girls have traits or stories that aren't presented in the TV series. For example, Lemmy has a background story about how she met Hiro as a little girl. Hiro was lost and Lemmy's parents helped him find his mother... meanwhile Hiro and Lemmy played together. Unfortunately, Lemmy had to move away, and she and Hiro weren't reunited until Lemmy moved back ten years later. Hoshina-san apparently has an Osaka accent in the game (I think she does in the show too) which she is rather sensitive about (this makes sense - her voice actress also does Kero-chan of Card Captor Sakura - another fine example of an Osaka accent... Kon'yanichiwaaaaa!!!). Her classmates make fun of her for it. These additions only serve to add greater depth to the characters.

The sample game images are from an extensive Chinese-language To Heart site called Akari's Home. If you'd like to see more images from the games (both PC and PSX versions), go there - it's easy to navigate even if you don't read Chinese.
